Shaping primary and community health services in Wales

The conference will:
- help to disseminate information about changes to primary and community health care in Wales
- help to share innovation in primary and community health care practice in Wales
- Provide opportunities for networking and forging new working partnerships.
Presentations will include:
- primary and community services delivery plan for Wales
- service integration and intermediate care
- supporting the independent sector
- information technology for community nursing
- LHB planning and delivering primary, community and mental health services
- rapid response services
- advanced nursing roles in primary care
- education and development in a primary care support unit
- developing the healthcare support workforce.
There is a fee of £15 for this event. We regret no invoices will be issued.
